Physical, Forms & Fees

  • Students participating in any sport at Liberty Middle School must have a Preparticipation Physical Evaluation (8th Grade Sports Physical) or 7th Grade Physical form (a separate sport's physical for 7th graders is not needed) and a Student and Parent Consent & Health Risk form turned in to the athletic office no later than noon of the day they plan to participate. Until these forms are on file, a student will not be allowed to participate in any activity, including tryouts and practice.

    If any 7th grader has a Physical/Vision Waiver, they would still be required to have a Physical or Sports Physical to participate.

    Students participating in Intramural Activities DO NOT need a physical.

    New Student Activity Fee Offsets Increased Expenses   

    Starting with the 2018-19 school year, all PLCS middle school students that participate in any sporting event or show choir will be asked to pay a one-time activity fee of $50.  This fee will be used to offset the increase cost of transportation to and from activities and to keep all middle school sporting events free of charge for families, students and community attendees.  Students that participate in multiple sports or show choir and sports will pay the fee only ONE time.  The fee will need to be paid prior to a student participating.  For any family on Free and Reduced, that is unable to pay the fee, waivers are available through the school counselor.
