  • Important Notice from the Health Office

In order for your child to be cared for appropriately and safely, the Papillion La Vista Community School District asks that all parents with children who have asthma, severe allergies, diabetes or seizures keep an action plan on file in the health office.  This includes students that self manage their asthma or diabetes.  All forms, including medication permits, are available for you to download from the district website www.plcschools.org under Health Services, or from your school nurse.    Please print the forms that you need and take them to your child’s doctor for updates. Send in the completed forms, with medications, to the school health office on or before the first day of school. Remember, new care plans are required each school year. Thank you for your cooperation.  Have a great summer!

A Note from your Middle School Nurse: Nebraska law requires all students entering 7th grade to have a complete physical (this physical also fulfills the requirements for a sports physical). Physicals should be performed after March 1st if not playing sports, and after May 1st if playing sports. Also required is a TDaP booster given after 10 years of age. Please call your Health Care Provider to make an appointment for your child’s physical and immunizations (call now- appointments fill up fast!). Once completed, we ask that you turn in your completed physical along with immunizations to your elementary school office or Liberty Middle School. Please email me at jtreadway@paplv.org with any questions.  Thank you!
