MAKE SCREENING PART OF YOUR MORNING ROUTINE!
Daily Screening Guidelines
All families will need to complete a daily screening prior to arriving at school each day.
It is VERY critical that if showing any symptom - stay home.
Do your part to keep everyone healthy and safe. Complete the daily screener honestly!
PLCS Partners with Children's Hospital & Medical Center to help families conduct daily screening for COVID-19
Through a partnership with Children's Hospital, all PLCS families will have access to a screening app that can be completed daily on your phone or any device. This screening tool will walk families through a series of questions and give you action steps that need to be taken based on your answers. Daily screening is critical in keeping all students and staff healthy and in school.

Student Handbook, Activity Handbook, NSAA Sport Consent Form
The Student Handbook, Activity Handbook, and NSAA Sports Consent Form are all online and available for student to access on the Parent Portal. All students need to sign off on the Student Handbook. Students wishing to participate in a Sport will also need to sign off on the Activity Handbook and NSAA Sports Consent Form.
Students will need to log into the Parent Portal and navigate to Student Verification.
The link is: parentweb.plcschools.org
When students log in – they use their Student ID and computer password. They only see the signatures section, so it is very swift and easy for students.
If students don’t remember their password –go to this link – reset their password and then navigate to Parent Portal – there is a short cut link on the password change site that takes them right to the Parent Portal after they change their password.
Students participating in any sport must have a Preparticipation Physical Evaluation (Sports Physical) or 7th Grade Physical form (a separate sport’s physical for 7th graders is not needed). Both Student and Parent Activity Handbook and NSAA Consent Form online signatures must also be completed for a student to participate.
Everything must be completed no later than noon of the day they plan to participate. Until these are done, and the $50.00 sports/activity has been paid, a student will not be allowed to participate in any activity, including tryouts and practice.
If a student has a Physical/Vision Waiver, they would still be required to have a Physical or Sports Physical to participate.
IMPORTANT: Parents: If you submitted your student’s Online Student Verification and did NOT sign off on the Activities Handbook or NSAA form, you will need to resubmit your Online Student Verification before your student can participate in any sport.
Incoming 7th graders be sure to schedule those 7th grade physicals, if you haven’t already. Physicals need to include a TDaP vaccine (and any others your child may need to be up-to-date), sports clearance (if your child wants to participate in a sport), and any action plans your child may need for school. Please ask your doctor to also check height, weight, vision, and hearing if possible.
Up to date immunizations are required for students to begin school (Reminder)
Parents can bring physicals to the school, fax them to 402-932-8452, or email them to our school nurse Ms. Caitlin Wilsey at email@example.com.
8th grade students wishing to participate in a sport will be able to use their 7th grade physical from the 2019-2020 school year as a sports physical for the 2020-2021 school year. If your child has any action plans, those would need to be updated for the current school year.
Health/Medical Action Plans (Reminder)
In order for your child to be cared for appropriately and safely, Papillion La Vista Community Schools asks that all parents with children who have asthma, severe allergies, diabetes or seizures keep an action plan on file in the health office. This includes students that self-manage their asthma or diabetes. All forms, including medication permits, are available for you to download from the district website at www.plcschools.org under Health Services, or from your school nurse. Please print the forms that you need and take them to your child’s doctor for updates. Send in the completed forms, with medications, to the school health office on or before the first day of school. Remember, new care plans are required each school year. Thank you for your cooperation.
If your student will be absent, a parent or guardian must call the school at (402) 514-3600 to report the absence or email Carrie Callahan at firstname.lastname@example.org. Calls should be made prior to 8:30am. A message can be left if the line is busy or if it is before or after school hours. Please note when informing the school of your child’s absence, you may be asked if this absence was directed by a health screener and/or for the child's symptoms.
Parents/Guardians should try to avoid taking students out of school for appointments if possible.
Appointments & Passes
If you need to pick up your child during the school day, you will need to call the school ahead of time or he/she may bring a note to the front office before school starts. Your child will be sent a pass allowing them five additional minutes to go to their locker and then come to the front office at the time specified. Parents will remain in their cars when picking up and dropping off students. Students will be released once they see their ride in front of the school. When dropping them back off, students will check back in through the front office and will be given a pass back to class. Please provide a doctor’s note as available.
